Submissions will only be accepted on CD (no MP3’s, flash drives, etc.) – and can be NO LONGER than three minutes in length. Submissions must also be
sent in via first-class mail only (express, priority, registered, certified and messenger mail are ineligible).
PLEASE READ THIS CAREFULLY! Entries must include a signed release form, found here: http://www.sirius.com/pdf/IWanttobeHoward.pdf, which includes entrant’s printed name, date of birth, home address, telephone number and signature.

How To Enter. To enter, submit one (1) audio compact disc recording of a radio demo no longer than three minutes in duration (a “Recording”) and
submit both the “Pre-Entry Affidavit of Eligibility, Publicity, Assignment, Release of Liability & Agreement” (“AFFIDAVIT”) and the entry form,
available at http://www.sirius.com/pdf/IWanttobeHoward.pdf, completed with the following information: contestant’s full name, dates of birth, and contact phone
numbers (day and night) (together, an “Entry”). An Entry must be submitted in the name of a single individual. Each Entry must be sent via first
